If this is your first visit to our physical therapists, we would like to help you know what to expect.

What to wear:

We ask that you wear comfortable clothing similar to what you might wear to a gym. Shoes are also important – a good pair of supportive athletic shoes or walking shoes are preferred.

What to bring:

  • Insurance card(s)

  • Payment for your visit. This may be for a co-payment, co-insurance or self pay. Payments may be made with cash, credit card, check or HSA card. ** NOTE:  We will make very effort to inform you of the amount that you will be asked to pay at the time we schedule your first appointment. However, it is YOUR Responsibility to know what your insurance will and will not cover and what your deductible and co-pay amounts are for Physical Therapy.

  • Photo ID

  • Written referral from your doctor (required by most insurance companies)

  • List of all medications that you are currently taking along with the dosage.

  • Any imaging reports (X-Ray, MRI etc.)

When you check in:

We have scheduled your initial visit 15 minutes ahead of when you will see your therapist. This time will be used for you to      complete our initial intake forms. Completing all forms is critical to helping your therapist understand your needs and to make sure that we can correctly bill your insurance provider.
