

Your First Visit
What to wear
We ask that you wear comfortable clothing similar to what you might wear to a gym. Shoes are also important – a good pair of supportive athletic shoes or walking shoes are preferred.​
What to bring

Insurance card(s)

Payment for your visit. This may be for a co-payment, co-insurance or self pay. Payments may be made with cash, credit card, check or HSA card. ** NOTE: We will make every effort to inform you of the amount that you will be asked to pay at the time we schedule your first appointment. However, it is YOUR Responsibility to know what your insurance will and will not cover and what your deductible and co-pay amounts are for Physical Therapy.

List of all medications that you are currently taking along with the dosage.

Any imaging reports (X-Ray, MRI etc.)​
What to expect
During your first visit we will:

discuss your problems and goals

perform a physical, movement-based examination

discuss our findings

develop a plan of care with you

start treatment

give you access to your online portal
When you arrive
We have scheduled your initial visit 15 minutes ahead of when you will see your therapist. This time will be used for you to complete our initial intake forms. Completing all forms is critical to helping your therapist understand your needs and to make sure that we can correctly bill your insurance provider.
